Implementing TLS secure transmission for medical information

hello,I have some problems in TLS secure transmission with DCMTK and OpenSSL.though,I knew the toolkit surports the TLS,I read the resource code and found nothing about exchaging/verifying the certifications,and encrypting/decrypting the data for transmission.In the document, the item "--cipher" means " add ciphersuite to list of negotiated suites" and all secure measures are put in the transport lay. who can tell me that how DCMTK with OpenSSL makes the data for commission confidential, integral and secure?
