How DICOM viewer display the image within a dicom file.

I have a dicom file ,the stored image under test is (16,12,11, PR=0) with VR=OW.
I use findAndGetUint16Array() to get the raw pixel values ,and find out the max pixel value is 1287, and the min pixel value is 1008.
I have the following questions ,
(1) when dicom viewer displays this image on a common monitor ,what kind of method is used to process the raw pixel ,e.g use histogram equalization corresponds 1008 to 0 ,and corresponds 1287 to 4095 , or use palette? is the method for display different by using different viewer?
(2) when dicom viewer displays this image on a professional monitor , is the method for displaying different?

Can anyone give me some ideas ?
In which chapter of the dicom standard can I find the answer to the above mentioned questions?
