They don't necessarily make a difference, but they can make one.
For example, "storescp" from DCMTK (part of dcmnet) isn't interested in either and doesn't really look at them. On the other hand, "dcmqrscp" (part of dcmqrdb) chooses the directory it uses for dicom files based on this information. If you try to connect to connect with an unknown AE title to it, it will refuse the association since it doesn't know which directory to use.
