This is an error I get on building dcmtk 3.5.4 p 2_1 under macports
on Snow Leopard.
Please let me know if I can figure out the same. Do I have to reinstall
ZLib.
Undefined symbols:

we did not try original dcmtk release 3.5.4 (with patch) on Snow Leopard. I recommend you to download the current snapshot of DCMTK and compile that on Snow Leopard.
Configure DCMTK without threads. (./configure --disable-threads).
If you want to stick with 3.5.4, try re-installing ZLIB (however, I think the one coming with Snow Leopard should be fine) or try compiling with ZLIB disabled (./configure --without-zlib I think).
