My application creates a DICOMDIR with the following options,

dcmmkdir.exe +I +m +X -Pno.

Does any one have any idea what is the -Pno option?

I could not find any help related to it in documentation nor web.

Yep, I read it but can we use any type I attribute to build the DICOMDIR if some attributes like Patient Id or Patient Name or anonymized?

That's a good catch, my DICOM files are anonymized, there is no Patient Id nor Patient Name, hence I had to specify the +I option to generate one as without it it throws an error. How can I force to generate the same patient id?

Can anyone let me know if all the DICOM compatible CD/DVD should be having a DICOMDIR? I need to implement reading the DICOM files in my application from a CD/DVD, hence would like to know if this(having DICOMDIR's) is standard.
